Fortis Student Select Insurance!

Student Select health coverage was designed just for college students. You choose your doctors and hospitals. You select the deductible and payment mode that best meet your needs. There are no health questions and coverage is guaranteed.

STUDENT SELECT provides a $100,000 per condition maximum benefit  amount and a $1,000,000 lifetime maximum benefit amount. A deductible amount  applies each calendar year. Prescription Drugs are NOT Covered.

STUDENT SELECT GUARANTEES ACCEPTANCE TO ANY ELIGIBLE STUDENT. ANY UNDERGRADUATE (TAKING NINE OR MORE COLLEGE CREDITS) OR FULL-TIME GRADUATE STUDENT, ATTENDING AN ACCREDITED COLLEGE OR UNIVERSITY UP TO AGE 63, IS ELIGIBLE FOR STUDENT SELECT. THERE ARE NO PHYSICAL EXAMINATIONS REQUIRED AND NO MEDICAL QUESTIONS TO ANSWER.

Student Select guarantees renewability. Whereas most college plans only cover until you graduate or very shortly thereafter, you can renew Student Select as long as you need it . . . and, of course, pay your  premiums.

Student Select guarantees your satisfaction. If  you're not entirely satisfied, return the policy within 10 days for a full refund. You may also cancel at any time and receive a refund as described in the policy.

Student Select covers you year round. With Student Select, you're covered an entire year, not just during the school term. And, if for some reason, you have to leave school, Student Select will stay with you for  the remainder of the policy year . . . and then it's guaranteed renewable

Student Select travels with you. If you transfer schools, your coverage goes with you. Student Select is not tied to any one  school. There's no need to change coverage or reapply. You're covered when you  travel anywhere in the United States, its possessions, or Canada. You're never restricted to an HMO or PPO service  area.

Here's What's  Covered:

Physician office visits for illness or injury.

Physician  services including inpatient and emergency room  care, and surgical care including anesthesia services and postoperative  care.

X-rays and laboratory services for outpatient x-ray,  radioactive treatment and laboratory services. Covered services include one screening mammogram per calendar year for females 35 years or  over.

Emergency care including ground and air ambulance to the nearest  hospital able to treat the illness or injury.

$1 million lifetime protection - $100,000 per illness or injury.

Medical evacuation when medically necessary after illness or injury resulting in hospital admission. Evacuation will be to home country or to a  facility operating within the laws and standards of home country. Coverage is up  to $10,000 per lifetime

Semi-private hospital room rate with no limit on days.

Intensive care and coronary  care units with no limit on days.

Organ  transplants including lung, heart, heart/lung,  liver, kidney, cornea, and skin. Lifetime maximum benefit of $100,000 for  services from donor search through follow-up care for 12 months post  transplantation.

Inpatient and  outpatient surgery including reconstructive surgery for an illness or injury commencing while covered under the policy.

Physical medicine for outpatient treatment of vertebrae, disc, spine,  back, neck and adjacent tissues up to $750 per calendar year.

Home health care including services administered during a home  health care visit such as nursing care, intravenous drug therapy, and  occupational/physical respiratory/speech therapies. Coverage is up to 160 hours of visits per calendar year. A visit can be from 15 minutes up to four hours.

Medical equipment and supplies such as crutches, wheelchair and oxygen.

Rehabilitation and skilled nursing facility charges for up to 30 days  following illness, injury or hospitalization.

Temporomandibular (TMJ) or Craniomandibular (CMJ) Joint Dysfunction services  including diagnostic exam and x-ray, injections, physical therapy and surgical treatment up to $1000 per lifetime.

Complications of pregnancy including miscarriage, ectopic pregnancy and spontaneous premature delivery of a nonviable fetus if the pregnancy begins and  ends while the covered person is covered under Student Select. Also covered are medical conditions whose diagnoses are distinct from pregnancy but are adversely  affected by pregnancy such as diabetes and chronic  hypertension.

Repatriation benefit which covers preparation and transportation of the insured remains to his or her home country or country of regular domicile if death occurs while insured, and cause of death is a covered illness or accident. The repatriation benefit is $10,000 over and above any other maximum benefit  amounts.

Although this is a good  description of the important features of the Student Select plan, this is not the insurance contract and only the actual contract defines coverage. Benefits may vary by state and by the terms of the insurance contract. Please read your  policy carefully when you receive it.

What is not covered?

It is important to understand that Student Select is not designed to pay for injuries and illness that exist at the time your policy becomes effective. Expenses for these pre-existing conditions incurred during  the first 12 months of the policy are not covered in most states. Student Select  also does not cover normal pregnancy or childbirth; mental illness or substance  abuse, routine physical exams or eye care; intercollegiate sports injuries;  charges incurred outside the United States, its possessions or Canada. Other  exclusions as well as other covered services are listed in detail in the policy  or certificate you will receive when you purchase Student Select.
