• The Federal government outlined that Employer based health insurance must be guarantee issue for legitimate employer groups of two employees or more. Some State Insurance Departments have mandated coverage for self-employed groups of one. There are no midwest states with that mandate. In order to qualify as an employer, you must be able to prove full time employment to insurance companies with either a Schedule C tax form, or a UC-3/40 Employers Contribution and Wage report ( or comparable state form)
